Zyprexa is also referred to as Olanzapine. It is a typical antipsychotic that belongs to the thienobenzodiazepine class. It is mostly recommended for oral administration. Oral zyprexa should be administered on once a day schedule without considering the meals, generally, starting with 5 to 10mg with a target dose of 10mg/day within many days. And if dosage adjustments are indicated, they should occur at interval of not less than a 1 week, since the slow state of zyprexa would not be achievable for approximately a week in the normal patient.
When adjustments of the dosage are necessary either an increase or a decrease of 5mg QD are preferable. The efficacy in zyprexa was shown in a dose span of 10 to 15 mg/day in clinical trials. However, doses that are above 10mg in a day are not demonstrated to be more efficacious than the 10mg in a day dose. An increase to a dose higher than the target dose of 10mg in a day is perfect only after the clinical assessment. Zyprexa is not indicated for use in doses that are above 20mg per day.
The effectiveness of the oral Zyprexa 10mg per day to 20mg per day in maintenance of the treatment response in schizophrenic patients who had been stable on Zyprexa for about 8 weeks followed for a relapse has been demonstrated in a placebo controlled trial.
Dosing in special populations
The required starting dose is 5mg to patients debilitated, who posses predisposition to hypotensive reactions, which show a combination of factors that may result in slower metabolism of the olanzapine for example non smoking female patients who may be more pharmacodynamically sensitive to olanzapine.
Adolescent’s dose selection
Oral onzapine need to be administered on a once day schedule without considering the meals with a recommended starting dose of 2.5mg or 5mg with a targeted dose of 10mg/day. The efficacy in adolescents with schizophrenia was shown based on adjustable range of dose of 2.5 to 20mg/day in clinical trials with a mean modal dose of 12.5mg in a day. When adjustments on dosage are essential, dose increase or decrease of 2.5 or 5mg is recommended.
Dosage for bipolar disorder for the adults
Dose selection for the monotherapy, oral zyprexa need to be administered on a single day schedule without regard to the meals ,generally starting with 10 or 15mg. The adjustments to the dosage if indicated should occur at intervals of not less than 24 hours, reflecting the procedure in the placebo controlled trials.
Dose selection for Adjunctive treatment
When administered as adjunctive treatment to valproate or lithium , oral olanzapine dosing should generally start with 10 mg in a day without regard to meals.
